Computer Configuration Request
This form is used to submit a computer configuration request for a new or old computer. It will give you an overall assessment of the costs regarding this work and allows you to keep a record of what needs to be done on the machine.
A FOAPAL is required for submission.
Please note that the FOAPAL is not charged for desktops used by faculty and departmental administrators. In this case, it is required for submission to validate the authenticity of the request.
